Four Core Cases: A Simulation Curriculum for Pediatrics Residents

This simulation-based curriculum was developed by the Boston Combined Residency Program to address gaps in pediatric resident experience with acute resuscitation. Due to the relative rarity of pediatric critical events and training constraints (e.g., work-hour limits and increased attending involvement), many residents lack opportunities to develop proficiency in leading resuscitations and performing advanced life support interventions.

The curriculum consists of four structured, case-based simulation scenarios designed to improve resident competence in pediatric resuscitation algorithms, procedural skills, crisis resource management (CRM), leadership, and interdisciplinary communication. The included cases—status asthmaticus, status epilepticus, septic shock, and toxicologic cardiac arrest—provide longitudinal exposure to high-acuity clinical situations in a controlled learning environment. Through deliberate practice and debriefing, the curriculum aims to enhance clinical confidence, technical performance, and teamwork behaviors necessary for effective management of pediatric emergencies.

Target Learners 

  • Pediatric residents (PGY-1 through PGY-3) 
  • Learners at varying stages of training progressing from: 
  • Basic resuscitation skills (PGY-1) 
  • Integration of advanced airway and resuscitation techniques (PGY-2) 
  • Leadership and team management during crises (PGY-3) 

Equipment Needed 

Implementation of this simulation curriculum requires access to: 

  • Pediatric high-fidelity simulation mannequin 
  • Airway management equipment (e.g., bag-valve mask, intubation tools) 
  • Intravenous (IV) and intraosseous (IO) access supplies 
  • Cardiac monitoring and defibrillation equipment 
  • Cardiopulmonary resuscitation (CPR) equipment 
  • Medications typically used in Pediatric Advanced Life Support (PALS) 
  • Simulation space capable of supporting multidisciplinary team training 
  • Audio-visual capability for observation and debriefing (recommended) 

Estimated Time to Complete the Activity 

  • Individual simulation case: approximately 45–60 minutes 
  • Scenario execution: 15–20 minutes 
  • Debriefing session: 30–40 minutes 
  • Full four-case curriculum: approximately 3–4 hours total, depending on group size and depth of discussion during debriefings
